Cedar Crest Clinic Policies

Cedar Crest respects the right of privacy of all our clients. Confidentiality is observed and protected at all times.

We require at least 24-hours notice if you will be unable to keep your appointment. If you do not give us 24-hour notice, a fee of $50 may be charged to your account.

Copayments, deductibles, and any existing balances are due at the time of the appointment.

If you request the doctor or therapist to complete forms, you will be charged a fee of $25.00.

If you request copies of your child's medical record, you will be charged a fee of $10.00 for the first 10 pages, plus 15 cents per page thereafter to a maximum of $25.00.

If a check you write us is returned, a fee of $35.00 will be charged to your account.
